I, the undersigned, give permission to Manitoba Canola Growers Association to use my image, voice or both in any photographs, videos or audio recordings taken for Manitoba Canola Growers Association promotional purposes only.

In addition, I waive all claims to compensation or damages based on the use of my image, voice or both by Manitoba Canola Growers Association. I also waive any right to inspect or approve finished photographs, videos or audio recordings.

I agree that all such photographs, video, audio recordings and any reproductions thereof as well as all recording tape and digital files will remain the property of the Manitoba Canola Growers Association unless otherwise noted.

I understand that this consent is perpetual, that I may not revoke it and that it is binding on my heirs and assigns. In addition, I am competent in my own name insofar as this consent is concerned. Finally, I attest that I have read this consent form and fully understand its content.
